Original passport valid for at least 6
months from traveling date (with enough blank pages for visa)

For Non- American citizens, if applicable,
a clear copy of your green card - front & back.

2 recent
passport-size photos - White background with your name on the back of the
pictures.

Visa Application please
fill-out in INK, one application per person.

Original vaccination certificate for
Meningitis.

If you are not carrying a Muslim name, a
certificate from the Imam of your mosques indicating that you are a Muslim is
required.

Husband and wife, a copy of the marriage
certificate is required.

Children going with father or mother copy
of birth certificate is required.

Children going with any one of the parent, a notarized
permission letter of other parent is required.
For Ladies Traveling for Hajj or Umra
-
Women traveling alone (must be over 45
years) without husband, brother or adolescent son, require a NOTARIZED
permission letter (Authorization)
from the husband, brother, or
adolescent son, indicating that he has no objection and permits her to travel
for (Hajj/Umra) with group.
